The Western Sydney Local Health District HR Case Management – Complaints and Grievances Portal holds NSW Health employee personal information, such as your name and other personal and employment details, and your records relating to complaints and grievance management. Western Sydney Local Health District is required to comply with the Privacy and Personal Information Protection Act 1998 and the Health Records and Information Privacy Act 2002 in the handling of this personal information.

The personal information in the HR Case Management – Complaints and Grievances Portal is in an eHealth Managed Secure NSW Health tenant for Office 365 and Power platform, for use by employees of Western Sydney Local Health District. Your personal information is stored on secure locations in the cloud in Australia region.

How we collect personal information

When you complete this form and send it to us, we collect the personal information contained in the form.

Personal information means information an/or opinion about an identified individual (or an individual who is reasonably identifiable). If you do not provide us with your personal information, there may be some limitations to progressing the matter you notify us about.

Access to personal information

Information held in the HR Case Management – Complaints and Grievances Portal may be accessed by your managers and People and Culture staff at Western Sydney Local Health District. Access is only permitted for authorised purposes.

Your information held in HR Case Management – Complaints and Grievances Portal may also be available to authorised staff in other NSW Health organisations when appropriate. If you are employed by another NSW Health organisation in the future, the information may be accessed by your managers and the Human Resources staff of that organisation for authorised purposes.

You have a right of access to your personal information held in HR Case Management – Complaints and Grievances Portal, and to request the correction of any inaccurate personal information.

Western Sydney Local Health District uses your personal information for managing processes associated with your employment relationship with Western Sydney Local Health District and general human resources management, including:

Western Sydney Local Health District takes reasonable steps to verify information collected to ensure that, having regard to the purpose for which the information is proposed to be used, the information is relevant, accurate, up to date, complete and not misleading. Information on HR Case Management – Complaints and Grievances Portal records may not always be validated or approved when first collected, however may be reviewed and validated at the time of utilising the information if it is reasonable to do so.

Your personal information may be disclosed to our service providers and contractors where necessary to support the uses outlined above for purposes directly related to human resources management of the Western Sydney Local Health District workforce. It may also be disclosed to the individual who you have made a complaint about. We will not disclose it without your consent.

We accept complaints made anonymously or by using a false name (pseudonym). You can also ask that we do not disclose information about you (including your name) to the person you have complained about. We refer to this as ‘confidential information’. There are limitations on progressing anonymous or confidential matters.

Western Sydney Local Health District may also disclose your personal information where authorised or required to do so by law, including:

  • in response to a subpoena, summons, search warrant, or court orders
  • to a law enforcement agency if there are reasonable grounds to believe that an offence has or may have been committed
  • for purposes required by child protection, taxation and employment legislation
  • where the disclosure is necessary to prevent or lessen a serious and imminent threat to the life or health of you or another person.
